Standard Referral Procedures and Information

• Contact the hospital via the referring D.V.M hotline at 508-887-4988
• Complete the referral form using the button above. Once the form is completed it will automatically be submitted to medrec@tufts.edu
• Once the referral form is submitted, the veterinary professional who submitted it and the pet owner will receive a confirmation email confirming the successful submission of the referral form.
• Foster Hospital for Small Animals specialty services will review the patient’s referral and medical history, and our liaison team will reach out to the client to book a consult. 

If you are referring a patient for an emergency or urgent medical issue that cannot wait for an appointment, please call 508-839-5395, press option 3, and email records and imaging to er-icu@tufts.edu.

If you are referring a patient for necropsy, please follow submission instructions through Cummings School Veterinary Diagnostic Laboratory. Please do NOT instruct pet owners to arrive at Henry and Lois Foster Hospital for Small Animals without prior arrangement.
 

If the patient is hospitalized, please let your clients know that a deposit of 75% of the maximum estimated cost of the treatment plan will be required. The balance of the invoice is expected at discharge.
