Referring Veterinarians

The Clinical Nutrition Service is here to help referring veterinarians manage their complicated nutrition cases, as well as to provide an opportunity to enhance their overall nutrition knowledge. The service currently provides brief case or topic discussions to veterinarians free of charge as well as written consults and home-cooked diet formulations for a consultation fee.

To request a formal consultation, please fill out a Consult Request Form and have the pet owner fill out a Diet History Form. These forms, as well as the results of current diagnostic tests (bloodwork, urinalysis and ultrasound) and any other applicable medical records (such as a weight chart), should be faxed or sent to the Clinical Nutrition Service.
